- National identity papers of each spouse.
- Passports of each spouse. (Citizens of Mainland China should provide the travel permit )
- Relevant lab reports and treatment records from the past six months.

1.For marriages registered in Taiwan, please bring the actual National ID cards of both spouses.
2.For marriages not registered in Taiwan (without a spouse name on National ID), please bring the actual marriage certificate or household registration book

How to get to HOPE fertility Clinic
ViaTaoyuan Airport MRT
- From Taoyuan international airport, take the airport MRT (A12/A13) to Taipei Main Station (A1). Then walk for approximately 10 minutes. The total travel time is around 1 hour.
Taxi
- From Taoyuan International Airport, it takes about 45 mins to 1 hour, with an estimated fare of roughly NT$1200.
MRT
- MRT NTU Hospital Station, Exit 4.
- MRT Taipei Main Station, Shin Kong Mitsukoshi Z2 or exit M8.
